What is the abnormality here?

Spot Diagnosis X

This 53 year-old man was referred by his optician because of dropping of the right eyelid. However, the patient was unaware of any aigns or symptoms. On examination, the right upper lid was ptotic with small pupil. The lower lid was also higher than that of the left giving the right eye an apparent enophthalmos. There was no heterochromia. Direct questioning did not reveal any signs or symptoms of asymmetrical facial sweating or flushing during exercise.

A 14-year-old girl was admitted for investigation of the sudden onset of chest pain. She had no significant past medical or surgical history, did not use illicit drugs, and denied recent episodes of vomiting, coughing, or air travel. BP was 117/76 mm Hg; heart rate, 83 beats/min; respiratory rate, 16 breaths/min; axillary temperature, 36.9°C; and oxygen saturation, 95%.
